Photo Release Form

Click on the link above to print a copy of Messiah's Photo Release Form. Please complete one form for each member in your family and return to the church office. This form is for the use of Messiah congregation only.

This release gives Messiah permission to use your likeness in Sunday school newsletters, Messiah TV monitors, brochures, website articles, and, potentially, community newspapers. If you have questions, feel free to contact Sheryl Pearson, Parish Administrator.
